The session continues a patient case from an earlier block. The patient developed kidney failure and, after about a year on the transplant waiting list, started peritoneal dialysis. Two years later she received her first kidney offer: a deceased donor kidney from donation after cardiac death, with a KDPI of 70%, cold ischemia time of 17 hours, and terminal creatinine of 1.2. After transplant, on post‑operative day one she had hyperkalemia (potassium 6.7) and required dialysis. Her urine output stayed low and creatinine remained high. She was discharged on post‑op day three still needing dialysis.
